What s the etiquette on appointments that day? Does the bride pay for hair appointments?

    If you are requiring your bridesmaids to have their hair professionally done, then you must pay. 

    If you don't care if their hair is professionally done or not, you could say something like "My hair appointment is at 11:00. If you would like to have yours done as well, I would be glad to make you an appointment and find out prices."

    I believe if the bride requires the BP to get their hair done she should pay for it.

    If you are requiring professional hair and makeup then you need to pay for it.  If it's optional each girl pays for herself.
