In MI you just have to go with both birth certificates and fill out the form 30 days before hand and pick it up 3 days later.  You pay $20 (if I remember right) for the license and then $10 to get a copy of the certificate to change your name.

Ours sucks because we both have to go in in person & they are only open weekdays like 9-4 or something , so there is another 1/2 day of time off for both of us.  I'd rather use my vacation days for fun things!  why can't they have some Saturday hours? 

It's valid for 30 days.  There is something funky about the county, I think you have to get it from either the county you live in or the county you are getting married in.  Luckily that is the same county for us.
