Escort cards - 1 per guest or couple?

I believe that each person should receive there own but I was looking for confirmation on this. I've read online that sometimes the entire couple is listed on one. Of course, I can't find a single escort card from a wedding I've been in or went to.

We did per couple. Escort cards are usually done that way.

This.  Although mine said
Joe and Anna Smith vs. Mr. and Mrs. but i did one per couple and for those who were not married, they each got one.

Thanks all! I thought it was one per guest and my FI thought it was one per couple. I looked it up online and most sites I found said to do one per guest, but I was thinking one per couple would be better... less to make!
Anyway, after seeing all of you responses I think we'll do one per couple for married couples.

What about unmarried people who have been dating a while? Should they be on one or each get their own?
Also, single people who are bringing a date and have filled in the RSVP card, "& Guest". I plan on calling them to find out who they are bringing so that they can get their own card. Is this what is typically done?
