RSVP wording help!!

I am trying to create my RSVP cards and the venue I am having the reception at has different pricing for several different age groups and I have to let them know a count for each. Does anyone have any ideas on how to word this on a response card? We are having family style so there won't be a food selection option.

How many different age groups are there? I'm assuming it's for children? I would just send the rsvps as you normally would and then figure out ages afterwards. You should have a rough idea at how old the kids are but if you're unsure you can always call after you get the rsvp.

I just saw a response card that matched your need online and now I can't find it.  It stated the regular wording then at the bottom it stated _____ children (9-12yrs).  I'll keep looking to see if I find it again.

There are different prices for 0-2, 3-5, 6-9, 10-20 and 20 and up. I appreciate the help, thank you!!
