  I've never seen anything like that on a response card. I think if someone you knew was going to go into shock and possibly die from a food allergy, you'd already know about it :-)

  Nope, not giving them food options.  I don't think my tiny brain can handle the kind of organization involved in food options and seating charts.

  I just put  "Please let us know if you have any special dietary needs" at the bottom.  You don't need to elaborate.  They'll find a way to contact you, and obviously they know that by putting it there, you'll coordinate with the caterer.   People will let you know without it, but I think it's nice to ask.    (Assuming, of course, that you will be able to accomodate anything that comes up. If you offer, you have to be prepared to find a gluten-free or nut-free option or whatever.)
